Good Faith Estimates for FQHCs

At most practices, a Good Faith Estimate is a fee schedule lookup. At a health center, it isn't. Who owns the GFE at your health center?

Good Faith Estimates for FQHCs

Ask a Federally Qualified Health Center who owns Good Faith Estimates and you will usually get three answers. The front desk schedules the visit, but they don't price services. Billing prices services, but they work after the visit, not before it. Compliance wrote the policy, but they don't generate estimates one patient at a time.

The GFE simply lands on whoever has time that day. Often that means no one.

The requirement itself is simple enough. Good Faith Estimates come out of the No Surprises Act and its push for price transparency. Uninsured and self-pay patients are owed a written estimate of expected charges when they schedule care, or any time they ask for one. Insured patients using their insurance don't trigger one. Medicaid patients don't either. For most FQHCs, that means the GFE lives with the uninsured and self-pay slice of the population. At a health center, that slice is still thousands of visits a year.

Why Good Faith Estimates Are Harder at an FQHC

At most practices, a Good Faith Estimate is a fee schedule lookup. At a health center, it isn't.

HRSA requires every health center to run a sliding fee discount program, so the right estimate depends on the patient's household income and family size, not just the service. Here is the catch. For a new uninsured patient, you don't know their sliding fee tier when they schedule. The deadlines for these estimates are often demanding as appointments scheduled less than ten days out require an estimate within one business day which means a significant amount of patients are due estimates in the same window that financial screening is taking place. The two tasks are entangled, and most workflows treat them as separate.

Two patients can receive the same service at the same site and owe very different amounts. A generic estimate that shows the standard charge is technically a number. It is not what the patient will actually pay.

Sliding Fee Scale Estimates: A Two-Sided Bind

Getting the estimate wrong costs you in both directions. Estimate too low, and if the final bill comes in more than $400 over, the patient can open a federal dispute through the patient-provider dispute resolution process. Your organization may eat the difference.

Estimate too high, and you scare off a patient who already believes they can't afford to come in. For an organization built to serve people regardless of ability to pay, that is not a billing error. It is a missed patient.

Those are the real stakes of GFE accuracy at a health center that can affect both the patient and the bottom line.

How Calvient Handles GFEs and Sliding Scale Discounts

Calvient now generates Good Faith Estimates with sliding scale discount logic built in. The workflow is short. Capture the patient's income and household size, assign the sliding fee tier, and apply the discount. It's simple enough that delivering the estimate to the patient while they are still on the scheduling call is a repeatable phenomenon.

Because Calvient already handles intake, eligibility, and patient communication, the estimate is not a separate system someone has to remember to open. It lives in the same workflow the front desk is already using.

There is a compliance benefit too. Every estimate keeps its paperwork. The income, household size, and any documentation the patient provided stay tied to the patient's record, next to the discount they justified. When an HRSA site visit or an audit asks why a patient received a discount, the answer is already in one place. Nobody spends a week reconstructing it from scanned files.

The Bottom Line

The patients get a number they can trust before they walk in. The front desk gets a task that can be performed instantly, and the question of who owns the GFE finally has an answer.
